How to Get YOUR Medical Marijuana Card

STEP 1 | SCHEDULE AN APPOINTMENT TO GET CERTIFIED AND GATHER MATERIALS

Make an Appointment with our Qualified Clinician:

  • Only a qualified clinician can initiate your application by certifying for the Department that you have a medical condition that qualifies you for a medical marijuana registration certificate.
  • If your physician decides to certify you for a medical marijuana registration certificate, he or she will ask you for a number of things including:
    • A valid e-mail address: This should be an email address that you are comfortable using in connection with your medical marijuana registration, as it will be the primary method the Department will use to communicate with you.
    • A primary telephone number:This should be a personal telephone number that the Department can use to contact you about your medical marijuana registration.

Step 2 | Register Online

Prior to attempting to complete the online application, you are strongly encouraged to access and refer to the Patient Quick Reference Card (pdf) and instructions below. These will assist you in completing the application process.

IMPORTANT: To complete the application, you must have specific documents and other items in a digital format ready for upload. Please review the information below carefully before beginning the online application process. If you need additional assistance uploading your documents into the system, please contact your chosen dispensary facility.

If you are a patient seeking to register with the Department’s Medical Marijuana Program you must:

Submit to the Department:

  • Proof of Identity (see examples)
  • Proof of Connecticut residency (see examples)
  • $100.00 registration fee (checks/money orders should be made payable to “Treasurer, State of CT”)

You will be able to upload these documents and pay the fee when you submit your registration application online. If you need assistance uploading any documents into the system, please contact your chosen dispensary facility.

Register Your Primary Caregiver, if Applicable

  • If your physician certification indicates a need for you to have a primary caregiver, you must register a qualified caregiver before the Department will issue you a registration certificate. The Department will not register a patient who needs a primary caregiver until the caregiver’s application is completed and approved.

Step 3 | Verify Email

  • You will receive an email from the DCP & Connecticut Medical Marijuana Program noting that they have received your application and are reviewing it.
  • You will receive a temporary card within 3-6 weeks via email.
  • Your physical card will be sent to you via mail 30-60 days after you receive your temporary card.

During a 10-15 minute virtual appointment, a CannaHealth qualified clinician will evaluate your ailments and answer any questions you may have about medical marijuana treatment. If qualified, the clinician will certify you directly within the Department of Health Medical Marijuana Registry following the appointment.

Step 2 | Register Online

The process of applying for/renewing a DC medical marijuana card has changed somewhat during these challenging times. You must have an email address and a credit/debit card with which you can pay online. Most people receive their card in 1-2 weeks after applying. You will need to obtain a recommendation from a health care practitioner first and then go to https://abra.dc.gov/node/1498666 

The application must be completed all at once, so please ensure you have everything you need, which will include:

  • A picture of your head and shoulders against a white background (only religious head-coverings are permitted by the Dept. of Health) 
  • Either a REAL ID (see photo above), OR
  • Two documents that show your current DC address:
    1. Driver’s license (if it has your current address) or
    2. District I.D.
    3. Current, unexpired Motor vehicle registration
    4. Voter’s registration
    5. Utility bill (not phone) within the last 60 days
    6. Current pay-stub that shows address, DC withholdings
    7. A signed lease/deed with your name and signature, your landlord’s signature, DC address, and date, AND
    8. Your electronic recommendation number from your healthcare practitioner
  • Upload your proofs of residency, photo ID and Medicaid or SNAP card (if applicable) onto to your computer/phone
  • Go to https://abra.dc.gov/node/1498666or Google search: DC Marijuana Applications
  • Choose from the list of healthcare practitioners and click on the appropriate link (Physician, Nurse Practitioner/APRN, Naturopath, Physician’s Assistant or Dentist)
  • Complete online form
    1. Enter recommendation number from a healthcare practitioner
    2. Click on the Application type (Initial or Renewal)
    3. Complete info as listed
    4. Photo- Upload your picture
    5. Photo ID – upload your driver’s license or state ID
    6. Proof of residency (all addresses must be the same)
      1. If you have a REAL ID with your current address you can upload it as a Photo ID and again for proofs 1 and 2
      2. If you do not have a REAL ID, you may use your driver’s license/state ID for both the Photo ID and one proof of residency
  • Enter one of the items listed in 1c above as your second proof of residency. 
  1. Reduced application fee (if you have Medicaid or SNAP) you may qualify for a reduced application fee ($25 instead of $100). Click “Check here if you qualify for a reduced application fee” and upload an appropriate picture
  2. Enter your healthcare practitioner’s name
  3. Complete Signature and click that you “agree to the terms and conditions”
  4. Click Submit

Step 3 | Verify Email

  • You will receive an email from the Alcoholic Beverage Regulation Administration (ABRA) noting that they have received your application and are reviewing it.  You will receive a payment link within the week.
  • When you receive the payment link, use a credit or debit card to pay the fee ($100 or $25 if eligible for reduced fee).
  • You should receive an email in the mail within two weeks. This will serve as proof of your license. The Alcoholic Beverage Regulation Administration (ABRA) will send you a card once the COVID19 crisis passes.

STEP 1 | SCHEDULE AN APPOINTMENT TO GET CERTIFIED AND GATHER MATERIALS

During a 10-15-minute virtual appointment, a qualified CannaHealth clinician will evaluate your ailments and answer any questions you may have about medical marijuana treatment.  If qualified, the clinician will certify you directly within the Department of Health Medical Marijuana Registry following the appointment.

You will need the following:

  • Electronic copy of a valid US government-issued photo ID and proof of Maryland address  
  • Electronic copy of a clear, recent photo  
  • Last four digits of the patient’s Social Security Number  

Please note: In lieu of a recent photo, Hospice patients may submit a letter from the Attending of Hospice Record (AOHR) provider on hospice letterhead

Step 2 | Register Online

  • Visit the MMCC Registry website and click the “Create Account” link under the “Patients” heading
  • Complete the “Before You Begin” section to review and confirm that you have the required documents and information ready for upload

Complete the online application form, uploading the requisite files.

Step 3 | Verify Email

  • After submitting your online application, you will receive a verification email  
  • Click the link supplied in the verification email to verify your email address  
  • You will receive another email after MMCC has reviewed your application, indicating it has been approved

Step 4 | Optional Final Step

Once approved by the MMCC, you can do the following:

  • Select a Caregiver if applicable by logging into the MMCC Patient Registry
  • Visit a registered provider to obtain a valid written certification

Purchase a Patient ID card (optional) for a fee of $50.

Step 2 | Register Online

Pennsylvania residents seeking certification for medical marijuana must create a patient profile with the Department of Health here to receive a patient ID number.   At the appointment, patients approved for certification provide the ID number to the physician and an electronic notice of the patients’ certification for medical marijuana treatment is submitted to the Department of Health. 

Step 3 | Verify Email

After the doctor certifies you in the system, return to the Registry and pay for a medical marijuana ID card.  The Department of Health will mail the card and you can begin purchasing marijuana legally from any Pennsylvania dispensary once you have your card in hand!
